The Omak team of CHRs has over 20 years of experience serving their community. Quail and Delaina visit clients to take vital signs and do overall health check-ups in between doctor appointments, help the person understand what their doctor has prescribed or suggested as treatment for various health issues, offer medication management and inventory, assess for environmental hazards such as fall risks, provide health coaching for the client and anyone else in their household on specific issues, and call in Nurse Farnsworth for more urgent medical assessments. They coordinate with local hospice and hospital services as needed to make sure their clients' needs are handled competently and professionally. Quail is also certified to assist people with Medicare enrollment.

Raelyn is the transporter for the Omak area, taking non-critical and medically stable clients to and from various medical appointments locally and as far away as Spokane and Wenatchee as needed. She is not only a great driver, but also a sympathetic listening ear for people who may have little social interaction.

The team cares deeply about their community and often fills other roles like cooking for funerals, gathering food for those in mourning, or delivering boxes of groceries from Second Harvest. They also serve as "first responders" for health emergencies such as the recent HantaVirus cases found on the reservation and the growing sexually transmitted infection testing that is needed.
