Our Community Health Representatives work in each district to provide at-home services such as wellness checks, some types of prescription delivery and health coaching. We can also arrange transportation to medical appointments if needed.

Communication is how we think, feel, and express our ideas, wants and needs. It's how we interact with the world. Several events can affect speech and language abilities throughout a lifetime from prenatal exposure to drugs and alcohol to traumatic brain injuries and listening to loud music as an adolescent or adult to strokes or dementia at a later age. Culturally sensitive assessment and screening is the first step in getting the help needed to improve communication. 
Our services may include:
  • ​Identification of speech/language difficulties
  • Screenings to determine type and severity of issue
  • Evaluation and assessment
  • Referrals as needed
  • Recommendations on rehabilitative techniques
  • Therapy, including teletherapy for individuals and groups
  • Consultations
  • ​Home visits
*Referrals from a doctor or nurse practitioner are required to receive speech and language therapy,  but if you have questions about what services are available or want to consult with Rhonda before seeing your care provider, give her a call. Serving people from newborn to elderly.
